URGENT PRESCRIPTION ORDERING INFORMATION

To reduce the risks of spreading Coronavirus for patients and staff we will NOT accept paper prescription requests until further notice.  We do not want patients entering the practice exposing themselves or our staff to risk of infection. You should request your prescription in one of the following ways:-

Via the Patient Access NHS App if you are signed up already

Email us at annandale.mc@nhs.net

You need to decide where we should send your prescription electronically. It can be to any pharmacy. You will not be able to collect your prescription from the Reception desk - please let us know your pharmacy preference. 

In some circumstances during this pandemic we will accept telephone requests for those unable to use the alternatives above and who do not have anyone available to help them, but you will still need to have a nominated pharmacy.

Named GP

From 1st April 2015 onwards Practices were required, under the GMS contract, to allocate a named accountable GP to all patients, including children.

All patients of Annandale Medical Centre already have a named GP allocated to them upon registration. Your named GP is your registered GP however this does not stop you from seeing any of the other GP's within the Practice.  If you are unsure of who your named GP is details can be found on your repeat prescription form, on the letter received from the Registration Authority when your registered or by contacting the surgery.

New patients of the Practice will be informed of their named GP on receipt of their registration form.  This will be told to you within 21 days of you registering with the Practice.

If you were registered with us before 1st April 2015 and were aged over 75 or took part in the Unplanned Emergency Admissions scheme you will have already been notified in writing of your named accountable GP – this named GP will remain the same and will not change.

Should you express a wish to change your named GP please write to the Practice Manager requesting the change.

Please remember that although you have a named accountable GP you can see and discuss any health concerns with any GP in the Practice and are encouraged to do so.
